Agreement Period: I understand this agreement is for a minimum period of 1* year(s). I agree to receive and pay for the services as outlined above. If for any reason I am not able to fulfill the 1* year obligation associated with this agreement, I agree to pay an early termination fee equal to 75% of the remaining contract value, unless original contract states otherwise.
I hereby authorize Green Clean USA to automatically withdraw funds (Electronic Funds Transfer) from my Debit/Credit card for the scheduled treatments outlined above. The entire amount due on my account will be charged after service has been rendered. This authorization will be in effect until written notification has been received. Cancellation of Easy Pay does not cancel the service program.
CONTINUOUS SERVICE: Green Clean USA services are provided on a continuous and auto-renewal basis. Green Clean USA reserves the right to increase the cost of its services at the end of each 12-month period. Either party may cancel this agreement in writing and owner/tenant agrees to pay an early termination fee equal to 75% of the remaining contract value, unless original contract states otherwise. Right of Rescission. Customer may rescind this Agreement within three (3) calendar days of enrollment by contacting Green Clean USA by phone or in writing. This Agreement is not legally binding until the rescission period has expired OR the first service has been completed whichever happens later.
*A one year contract minimum but may be stated otherwise during signup in terms of one, three, & five year increments. The exception to this term is for a one-time only cleaning or a quarterly cleaning without contract selection.
